Photo ID is required for all donors. Potential donors must be: in good health; have a hemoglobin level that meets FDA standard; must weigh at least 110 lbs; must be at least 17 years old; must not
have donated blood in the last 8 weeks; must not have visited India within the last year.

All donors are required to complete a health questionnaire and blood safety form during a confidential interview.
